Background Imperforate hymen is a unique anomaly, there is obstruction of vaginal outflow in the vaginal introitus due to abnormal development of the epith elial layer that is connected to the hymen tissue and then total obstruction occurs. Hematometra and hematocolpos are typical signs of an ultrasound examination to make an diagnosis, in addition to genitalia examination of the vaginal introitus.

Case Summary 16-year-old female adolescent with imperforate hymen, with complaints of lower abdominal pain and feeling enlarged, vital signs are blood pressure 110/70 mmHg, pulse 80x / m, breathing 18x / m and temperature 36.5C . On physical examination, the suprap ubic-umbilical quail inspection appears to be rather large, on examination of the vulva and vaginal gynecological status, hymen covers the entire vaginal introitus, redness, hymen buldging (+) (Figure 2), blood ( -) labia major: normally, labia minor: within the normal range, clitoris: normally, ultrasound examination suggestsedhematometra and hematocolpos (Figure 3). Then an cross-incision hymnetomywere performed
at the UNDATA Hospital in Palu. Providing antibiotic therapy, postoperative analgesics were also performed. Furthermore, evaluation of 4-6 weeks is necessary to assess success and complications after the operative action.
